With the outbreak of COVID-19 and the rising concern for student's mental and physical health, in order to formulate the required areas for evaluation which is used for effective interventions, based on research literature and theoretical foundations, different emotional, communication, functional, lifestyle, perceptual and cognitive issues were considered as the main areas for developing a self-assessment survey.

In the first part of the questionnaire which evaluated positive and negative affect, from among different kinds of positive affect, feeling secure, respectable, valuable, gratitude and happiness were reported to be at a more desirable level for students. On the other hand, from among negative affects, nostalgia, feeling stressful, bored, fatigue and anger were reported as the most undesirable cases.

Students' reports on the quality of sleep, nutrition, and physical activity indicate that most of them experienced a relatively healthy eating style, but had developed bad sleep habits and much lower exercise.

Out of the extracurricular activities list, including cooking or confectionery, spiritual rituals, learning a new skill, strengthening a foreign language, virtual communication with friends, watching movies or series, listening to music, playing an instrument and painting, based on students reports, watching movies and series, listening to music, and virtual communication with friends earned the highest scores.

Regarding student's perceived support resources in areas like confabulating, seeking guidance and counseling, help with homework, making important decisions, solving problems, asking and resolving doubts, getting a sense of support and understanding, talking about topics of interest, talking about unpleasant feelings and worries, according to students' report, important decisions, problem solving, and seeking guidance were among the areas in which they rated having better support.

These data helped the school counselors to schedule the best interventions possible including organizing individual and group meetings with families, teachers and the students, and also psychoeducational content production, in order to meet the areas which were considered undesirable according to student's self-assessments.
